Terms & Conditions

Using this website
This website provides general information about Swift Rehabilitation, Inc., its services, educational resources, and opportunities to work with or partner with the practice. These terms govern website use. Your treatment, billing, consent, and employment documents, when applicable, are provided separately.
Health information and emergencies
Website articles, exercise examples, the automated care guide, and printable worksheets are educational. They do not diagnose a condition, prescribe an individual treatment plan, or establish a therapist–patient relationship. Ask your clinician which activities and forms are appropriate for you. Do not use this site to delay medical care. Call 911 for a medical emergency.
Appointments, services, and coverage
An appointment request is not a confirmed visit. Swift must confirm availability, the appropriate service and location, and any required documentation. Insurance eligibility, benefits, authorizations, and patient costs are reviewed individually. Website information does not guarantee coverage, treatment outcomes, or availability in every area.
Patient forms and privacy
The patient worksheets on this site are fill-and-print tools. Their answers are not submitted to Swift through this website. Bring completed paperwork to your appointment or ask Swift for an approved secure delivery method. Do not send health information through social comments, the careers form, or ordinary website inquiries. Read our Website Privacy Policy for details. Swift’s clinical Notice of Privacy Practices is a separate document.
Exercise and outcome-measure resources
Use exercise resources only as appropriate for your health and your clinician’s instructions. Stop an activity that causes worsening pain, dizziness, or new symptoms and seek advice. External questionnaires may be subject to copyright, permissions, or licensing. Their owners’ terms apply. An online score or worksheet does not replace a clinical evaluation.
Applications and acceptable use
Career submissions are expressions of interest, not job offers. Supply accurate information and only material you have the right to share. Do not submit patient records, malicious files, unlawful content, or another person’s private information without authority. Do not attempt unauthorized access to the website or application inbox, interfere with its operation, or impersonate another person.
Content and third-party links
Swift branding and original website materials belong to Swift or their respective rights holders. You may print the patient worksheets for your own care. Third-party resources, social networks, maps, booking services, and copyrighted questionnaires remain subject to their own terms and privacy practices. Linking to a resource does not mean it endorses Swift.
Availability, corrections, and changes
Website access may be interrupted and content may require correction. We may update these terms and display a revised date. Nothing here waives rights or obligations that cannot lawfully be waived. Contact Swift if you notice an error or have a question about these terms.
