What this policy covers

This policy describes information handling for this Swift Rehabilitation website. It is separate from the HIPAA Notice of Privacy Practices that governs the practice’s clinical use and disclosure of protected health information. To obtain that notice or discuss a medical-records request, contact Swift directly.

Information you choose to provide

The careers form collects your name, email, phone number, professional role, preferred state and work arrangement, licensure information, service area, experience, availability, consent, and any optional PDF résumé. These submissions are stored through the site’s hosting storage and made available in an inbox restricted to the site owner. Swift uses them to review your interest and communicate about recruitment.

Appointment links open Swift’s separate booking service. Information entered there is handled by that service and Swift’s applicable intake practices. Calling a phone number or opening a map or social link takes you outside this website.

Patient worksheets: fill, print, and bring

The patient intake, privacy-notice acknowledgment, and symptom worksheets run in your browser. This implementation does not send their answers to Swift, save them in website storage, or include them in the careers inbox. Typing a name is not an electronic submission or verified signature. Print the completed worksheet or use your device’s Save as PDF option, then bring it to your visit.

Your browser, device, autofill features, printing service, or saved PDF may retain information independently. Use a trusted device, keep printed or downloaded copies secure, and clear the worksheet when finished. Ask Swift for a secure delivery method if you cannot bring paperwork in person.

Technical data and service providers

The hosting and authentication infrastructure may process technical details such as IP address, browser or device information, requested pages, access logs, and sign-in information to operate and protect the site. Private administration areas require sign-in.

The site currently loads web fonts from Google Fonts, which can receive technical request information when fonts load. We have not added advertising pixels. Optional Google Analytics loads on the homepage after you allow it. It measures homepage visits and clicks on call and appointment buttons; clicks do not confirm completed calls or appointments. Google may process cookie identifiers, IP address and device information. Our custom events do not include form answers, names, emails or phone numbers. Patient worksheets, clinical articles and other pages are excluded from this initial setup. Your choice is saved in this browser; change it through Analytics preferences in the homepage footer. Hosting or sign-in services may use essential cookies. Social and video profiles are linked rather than embedded feeds.

Sharing and access

Site data may be processed by hosting, storage, authentication, and other service providers needed to operate the website. Recruitment records are not published in the public pages. Information may also be disclosed when required by law or needed to protect lawful rights and security. This website does not include a feature that sells submitted information or shares it with advertising networks.

Retention, correction, and removal

Applications remain in the recruitment storage until Swift removes them; automatic deletion is not configured in this version. Contact Swift to request access, correction, or deletion of information you submitted. Requests may require identity verification and may be limited by applicable retention obligations. This website does not maintain a clinical patient portal or a medical-records archive.

Your choices and external services

You can browse available pages without submitting an application. Do not include Social Security numbers, financial account details, patient records, or unnecessary sensitive information in a résumé. Review the privacy practices of external services before providing information to them. Do not post health details in social media comments or messages expecting them to be a secure clinical channel.

Security and updates

Access controls are used for the application inbox, but no internet system can promise absolute security. This website is not represented as a HIPAA-compliant patient portal. We will update this policy when the site’s information practices change and revise the date above.
