Begin with participants and their routines
Tell Swift about your program, location, daily schedule, and the types of support participants need. The initial conversation helps us understand what services and therapist availability may fit your setting.
Plan the referral process
Agree on a contact person and a secure way to share referrals and clinical documentation. Each participant still needs an individual evaluation and any applicable coverage or authorization review. A facility partnership does not automatically establish eligibility for every participant.
Make therapy part of the day
Discuss space, coordination with meals and activities, and how caregivers or staff can communicate relevant changes. The goal is a workable process for your center and individualized rehabilitation for participants.
Keep communication practical
Ask how the therapist will share relevant recommendations, coordinate with the care team, and arrange follow-up. Swift welcomes partnership inquiries from facilities throughout Maryland and New Jersey, with availability discussed for each location.
