Start with your own goals
Think of two or three everyday activities you would like to improve: reaching a cabinet, walking to the dining room, getting dressed, or taking part in a community activity. Write down what is difficult and what a useful improvement would look like to you.
Gather what you already have
Have your medication list, insurance card, relevant referral or discharge paperwork, and any existing exercise instructions available. You can fill out our intake worksheet in advance and bring a printed copy. Ask our team how to return records securely if needed.
Prepare a practical space
Choose comfortable clothing and tell the team about access instructions or any pets. Keep your usual mobility device nearby. You do not need to buy exercise equipment before the evaluation; ask the therapist what is appropriate.
Make room for questions
Ask what the evaluation will involve, how your goals will be tracked, and what you should do between visits. If a caregiver helps with daily activities, discuss whether it would be useful for them to attend. Confirm scheduling and coverage with Swift before the visit.
